Go to the Athletic Website to get all important documents that are required to be a part of a sport at GBHS.

If you are interested in participating in sports in the 2010-2011 school year please read all of the Parent letter and Athletic Handbook and follow the steps below to print off the appropriate forms.
Step One: Read Parent Letter and Athletic Handbook
Step Two: Print Athletic Clearance (Health History Questionaire), Parent Permission
and Consent to Treat Form
Step Three: Print Agreement Regarding Use of Steroids
Step Four: Print Health Emergency Information
Step Five: Have Physician complete, sign and date Health History Questionnaire
